2015-2016 TRANSPORTATION FORM
If your child will be attending a daycare program before or after school, a new Child Care Form must be filled out each school year. Information does not carry over from year to year. This information holds true even if your child is attending the YMCA Program before/after school at Oaks Elementary. Due to a change in the PA State Child Protective Services Laws, effective July 1, 2015, there are new clearance requirements for all VOLUNTEERS who would like to assist in a classroom, library, go on a field trip, assist with the School Store or with building activities/events. Please click on Volunteer Information on the district's homepage located along the right hand side for additional information regarding the requirements/clearances you must obtain before you may volunteer.

MODIFIED KINDERGARTEN SCHEDULE
AM Kindergarten -- 11:00 AM - 12:45 PM
PM -- Kindergarten -- 1:55 PM - 3:40 PM
Administration Of Medication During School Hours:
Medication should be scheduled around school hours if possible. Medications orders are required from a physician for prescription and over-the-counter medicine, as well as herbal remedies.
All medications administered in school require the completion of the “Authorization for School Medication Administration” form found on the district website under Forms, Health. A new prescription is needed every school year for medicine that is taken on a long term basis. No medication will be administered by the nurse without proper physicians orders on file.
